Evergreen Pelvic Health is an out-of-network provider, which means our care is guided by your needs—not insurance requirements. This allows us to provide one-on-one, hour-long sessions focused on personalized, evidence-based treatment without the limitations that can come with insurance-driven care.

Appointments are $190 per session. Payment is due at the time of service, and we are happy to provide a detailed superbill that you can submit to your insurance company for potential out-of-network reimbursement. Coverage varies by plan, so we recommend contacting your insurance provider to learn more about your out-of-network benefits. We do accept HSA and FSA accounts.

We'll discuss your goals, medical history, daily activities, and symptoms before deciding together what the evaluation will include.
